Basic Life and Accidental Death & Dismemberment Insurance
HonorHealth automatically provides basic (employer-paid) life and accidental death and dismemberment insurance at no cost. You must designate a beneficiary for this coverage in Staff Member Self Service. Instructions on how to complete this can be found HERE.
Voluntary life insurance
You have the option to enroll in voluntary life insurance plans for yourself and your eligible dependents to help you and your family prepare for life’s uncertainties. You may apply for this benefit at any time during the year; however, enrolling within 30 days of your hire or eligibility date eliminates the need to provide proof of good health. Voluntary life insurance benefits are administered through The Hartford.
As the employee, you're automatically designated as the beneficiary for any spouse and/or dependent life insurance coverage you elect. To apply for voluntary life insurance for you and your eligible dependents, please complete an application.

Short-term and long-term disability coverage
After one year of service, you may be eligible for salary replacement in the event that you're unable to work due to a non-work-related illness, injury or other condition. Information about short and long-term disability coverage will be mailed to your home around the time of your one-year employment anniversary.
