If you are injured or exposed to potentially dangerous substances, you need to immediately report the incident to your supervisor or Admin Rep who will need to complete the Supervisor Report of Injury. If the injury involved a sharp, i.e. needlestick, etc., the Sharps Incident Report will also need to be completed. The completed form(s) should be emailed to HonorHealth@ccmsi.com.

For medical treatment, report to HonorHealth’s Thompson Peak Employee Health location or call the triage number 480-323-1880 to schedule an appointment during business hours (Monday through Friday 7:30 a.m. – 5:00 p.m.). If you have a life-threatening injury, go to an HonorHealth Emergency Department (ED) or Urgent Care location.
